Swimming Lesson Consent Form

MEKENIFICENT CENTER is happy to offer the option of swimming lessons for adults, children and/or athletes. This form needs to be completed and submitted with a Proof of Payment. Please note that pool space and instructor availability are limited hence the reason we ask you to provide multiple options for times you would like to schedule your swim lessons.

Swimming Lessons

Adults - N$ 550 per month

Kids - N$ 450 per month

I understand that during the session, the reaction of my child's body, cannot be predicted with accuracy, thus risks may arise due to adverse changes. I understand and consent to any emergency medical treatment required during the session. I understand and agree to abide by all rules and health conditions required for use of the pool. I understand all needed equipment given above must provide by myself, I. the undersigned, my heirs and assigns, hereby, indemnify and holds harmless the MEKENIFICENT (its directors, employees and agents) from all legal responsibility for any death, injury or any illness created or sustained by any MEKENIFICENT activity and/or program.
